甘精胰岛素在老年糖尿病中应用特点

Application characteristic of insulin Giargine in old adults with diabetes

目的探讨甘精胰岛素在老年糖尿病中应用特点。方法226例口服降糖药血糖控制不理想的2型糖尿病患者分为老年组(107例)和成人组(119例),两组在原口服降糖药治疗不变基础上加用甘精胰岛素治疗。根据空腹血糖调整胰岛素剂量,FPG≤7.8 mmol/L为治疗目标,共12周。观察血糖控制、体质量、甘精胰岛素剂量及注射时间和低血糖事件发生情况。结果两组在BMI、FPG及HBA1C比较无统计学意义条件下,老年组甘精胰岛素所需剂量明显少于成人组(P<0.01),在早餐前注射甘精胰岛素夜间低血糖事件明显少于睡前注射(P<0.05)。结论老年糖尿病应用甘精胰岛素治疗所需剂量少于一般成人,以早餐前皮下注射为佳,可减少夜间低血糖发生。

Objective To discuss the application characteristic of insulin Glargine in old adults with diabetes. Methods 226 cases with inadequate glycemia by oral administrations were divided into the old age group （n =107） and the general adult group（n = 119） according to the age,the original oral administrations add with the insulin Glargine to unite the treatment in the two groups. The dosage of insulin was adjusted by FBG level, seeking a target of FPG ≤7.8 mmol/L, and the treatment lasted for 12 weeks. The blood glucose level and the BMI , and the daily dosage of insulin were observed. The relatings between the injection time of insulin and the hypoglycemia event was analyzed. Results Mean of BMI and FPG,and HbA1C were similar in the two groups, but the daily dosage of insulin in the old age group was significantly lower than that in the adult group（ P 〈0.01 ）. Injects the insulin in front of the breakfast the hypoglycemia event was significantly lower than that at night （P 〈 0. 05 ）. Conclusion The daily dosage of insulin Glargine in old adult with diabetes is lower than that in the general adult with diabetes. Inject the insulin in the front of the breakest is the better. It may reduce hypoglycemia at night.
