胫骨开放性骨折伴软组织严重损伤治疗探讨

Study of treating open tibial fractures acompanying local severe soft tissues injurie

目的探讨胫骨开放性骨折伴局部大片软组织严重损伤治疗方法。方法对我院2001年6月～2007年6月收治的胫骨开放性骨折伴局部大片软组织严重损伤52例治疗进行回顾性分析，创面分别行早期清创皮瓣转移治疗（早期组）和晚期扩创皮瓣转移治疗（晚期组），每组26例，比较两组术后并发症。结果早期组后期并发症7例，晚期组16例（P〈0．05，χ^2=6、315）。结论胫骨开放性骨折伴局部大片软组织严重损伤行早期清创皮瓣转移治疗是较为有效的治疗方法。

Objective To discuss therapeutic methods of open tibial fractures acompanying large local severe soft tissue injuries. Methods Fifty-two patients with open tibial fractures acompanying large local severe soft tissue injuries were retrospectively analyzed in our department from June 2001 to June 2007. Regarding 26 cases of early cleaning wound and flap transfer as early group, while 26 cases of late debridernment and flap transfer as late group, we compared their late complications. Results The early group showed late complication in 7 cases, the late one in 16 cases （ P 〈 0. 05, χ^2 = 6.315 ）. Condusions The treatment of early cleaning wound and flap transfer towards open tibial fractures acompanying large local severe soft tissues injuries is a more effective method.
