呼吸操对慢性阻塞性肺病患者肺功能和生活质量的影响 被引量:15

摘要 目的研究呼吸操对慢性阻塞性肺病(COPD)肺功能和生活质量的影响。方法用随机、对照的设计方法观察了55例COPD患者呼吸操治疗后对肺功能和生活质量的疗效。结果呼吸锻炼6个月后用力肺活量(FVC)由治疗前的(2.02±0.48)L提高到(2.53±0.55)L(P<0.05);1秒用力呼气容积(FEV1)由(1.28±0.57)L提高到(1.65±0.48)L预计值(P<0.01);FEV1/FVC由71.22±11.43提高到89.86±12.36(P<0.01);PaO2由治疗前(56.54±4.03)mm Hg提高到(71.04±4.63)mm Hg(P<0.01);PaCO2由(53.08±10.00)mm Hg下降到(52.11±11.05)mm Hg(P>0.05)。生活质量评分:治疗组呼吸操前总均分由(2.7±0.4)分下降到(2.1±0.4)分(P<0.05),对照组(2.8±0.5)分上升到(3.1±0.6)分(P<0.05)。结论呼吸操可使FVC增加,FEV1预计值增加,FEV1/FVC百分比增加,从而可以改善COPD患者的肺功能。呼吸操治疗后QOL评分下降,因而能提高生活质量;同时呼吸操治疗还可锻炼和增强患者的体力和活动能力,提高抗病能力,延缓疾病的进展,从而最终提升患者的生活质量。 Objective To study the effect of breathing exercises on pulmonary function and quality of life(QOL) in chronic obstructive pulmonary disease(COPD). Methods Therapy effects of pulmonary function and QOL in 55 COPD patients after taking breathing exercises were examined using randomized and controlled design methods. Results Forced vital capaciy(FVC) was increased from pre-therapeutic (2.02±0.48) L to (2.53±0.55) L ( P 〈0.01); forced expiratory volume in one second(FEV1) raised from (1.28±0.57) L to (1.65±0.48) L estimate value( P 〈 0.01) ; FEV1/FVC was improved from 71.22±11.43 to 89.86±12.36 (P 〈 0.01) ; PaO2 raised from (56.54±4.03) mm Hg to (71.0±4.63) mm Hg, but PaCO2 reduced from (53.08±10.00) mm Hgto (52.11±11.05) mm Hg (P 〈 0.01) ;QOL score of the group after taking breathing exercises was decreased from (2.7±0.4) scores to (2.1±0.4) scores ( P 〈0.05) and that of control group was increased from (2.8±0.5) scores to (3.1±0.6) scores ( P 〈 0.05). Conclusion Breathing exercises might increase FVC, FEV1 estimate value and FEV1/FVC so as to improve pulmonary function of COPD patients. The score of the group after taking breathing exercises is decreased, which suggests that it might raise QOL in patients with COPD. Otherwise, breathing exercises might reinforce the physical strength and locomotor activity,improve the disease-resistant ability, delay progress of the disease so as to raise QOL in patients with COPD.
