超声检测肝短静脉在诊断肝淤血中的价值

Clinical value of ultrasonographic detection of short hepatic vein in diagnosis of hepatic gore

目的评价超声检测肝短静脉(SHV)在诊断肝淤血中的价值。方法应用彩色多普勒超声对3组423例患者(1组:非肝淤血,肝静脉内径<10 mm;2组:非肝淤血,至少1支,肝静脉内径≥10 mm;3组:肝静脉内径≥10mm,肝淤血)的SHV进行观测,检查并记录SHV的位置、数目、管径、血流频谱。结果SHV检出率:1组192例检出SHV 60例,检出率31.3%,测及84支SHV;2组132例检出SHV 76例,检出率57.6%,测及88支SHV;3组99例检出SHV 90例,检出率90.9%,测及117支SHV。1支SHV检出率:1组42例(70.0%);2组64例(84.2%;3组66例(73.3%)。2支SHV检出率:1组12例(20.0%);2组12例(15.8%);3组21例(23.3%)。3支SHV检出率:1组6例(10.0%);2组0;3组3例(3.3%)。肝右后静脉检出率:1组18例,检出率9.4%;2组12例,检出率9.1%;3组33例,检出率33.3%。肝尾叶静脉检出率:1组60例,检出率31.3%;2组76例,检出率57.6%;3组81例,检出率81.8%。肝淤血组患者SHV内径增宽、SHV显示率较正常人高。结论肝淤血患者肝短静脉内径增宽,显示率增加,可为肝淤血的诊断提供帮助。

Objective To evaluate clinical value of ultrasonographic detection of the short hepatic vein in diagnosis of hepatic gore. Methods A total of 423 cases were divided into three groups,the short hepatic vein was checked by ultrasound. The location,number, diameter, blood flow spectra of short hepatic vein were studied. Results Detecting rate of SHV：84 SHV were discovered in 60 of 192 cases（31.3%） in group one,88 SHV in 76 of 132 cases （57.6%） in group two,and 117 SHV in 90 of 99 cases（90.9%） in group three. Detecting rate of 1 SHV：42 cases in group one（70.0%） ,64 cases in group two（84.2%） ,and 66 cases in group three （73.3%）. Detecting rate of 2 SHV：12 cases in group one （20.0%） ,12 cases in group two 2（15.8%） ,and 21 cases in group three （23.3 %）. Detecting rate of 3 SHV. 6 cases in group one （10.0%）, 0 cases in group two （0%）, and 3 cases in group three （3.3%）. Detecting rate of the right post SHV： 18 cases in group one （9.4%）, 12 cases in group two （9.1%）, and 33 cases in group three （33.3%）. Detecting rate of SHV of caudate [obe： 60 cases in group one （31.30%）, 76 cases in group two 2 （57.6%）, and 81 cases in group three （81.8%）. The diameter and display rate of SHV in group three were obviously higher than those in group one and two. Conclusion The diameter and display rate of SHV in patients with hepatic gore are increased,it is helpful for the diagnosis of hepatic gore.
