双水平气道正压通气呼吸机治疗慢性阻塞性肺疾病并发Ⅱ型呼吸衰竭疗效观察

BiPAP respiratory therapy on chronic obstructive pulmonary disease complicated with type-Ⅱ respiratory failure

目的观察双水平气道正压通气（BIPAP）呼吸机治疗慢性阻塞性肺疾病（COPD）并发Ⅱ型呼吸衰竭的疗效。方法将40例COPD并发Ⅱ型呼吸衰竭的患者采用完全随机设计方法，分为试验组和对照组各20例，对照组采用常规治疗，试验组在常规治疗的基础上加用BIPAP呼吸机治疗，所有患者分别在治疗前及治疗24h后测定pH值，氧分压（PaO2），二氧化碳分压（PaC02）和用力肺活量（FVC），并记录临床症状的变化。结果24h后临床症状和各指标的改善情况试验组明显好于对照组。结论BIPAP是治疗COPD并发Ⅱ型呼吸衰竭的一种较好的无创通气模式。

Objective To observe the effect of biphasic intra-airway positive air pressure （BiPAP） respiratory therapy on respiratory failure. Methods Forty patients suffering from chronic obstructive pulmonary Disease （COPD） were divided into control group and treatment group. The control group was treated with common therapy and the treatment group was treated with both common and BiPAP. PH, PaO2, PaCO2, and FVC were analyzed before and 24h after therapy, meanwhile changes of clinical symptoms were recorded. Results Symptoms of the treatment group showed more improvement than the control group 24h after therapy. BiPAP relieved respiratory muscle fatigue,which increased tidal volume, decreased respiratory rate and oxygen consumption. Conclusion BiPAP was an optimistic and non traumatic method for treating COPD with type-Ⅱ respiratory failure.
