慢性阑尾炎腹腔镜与开腹手术切除阑尾对机体免疫功能影响的比较

Effects of laparoscopic appendectomy and opening appendectomy on immune function in chronic appendicitis patients

目的研究慢性阑尾炎应用腹腔镜和开腹切除阑尾对机体免疫功能的影响。方法对50例需要手术的慢性阑尾炎患者随机分为两组,腹腔镜阑尾切除手术(LA)25例,开腹阑尾切除手术(OA)25例,测定术前24h、术后24h及48h静脉血清C反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)。结果两组术前24h各数值相比差异无显著性(P>0.05);术后24h及48h各数值均有不同程度的升高,但OA组的各观测指标均显著高于LA组(P<0.01)。结论在慢性阑尾炎手术上,LA比OA引起机体免疫功能的变化较小。

[Objective] To explore the effect of laparoscopic appendectomy and opening appendectomy on patients＇ immunity who had chronic appendicitis. [Methods] 50 cases with chronic appendicitis were divided into two groups randomly： 25 cases were operated by laparoscopic appendectomy （LA）, and the other 25 cases were operated by opening appendectomy （OA）. C-reactive protein （C-RP）, IL-6, IL-8 in serum from venous blood were tested at 24 h before operation, 24 h and 48 h after operation. [Results] The data of the two groups before operation was not changed significantly after operation. It was demonstrated that data of the two groups increased in some extent postoperatively. While the contents of OA increased markedly than that of LA postoperativety （P 〈0.01）. [Conclusion] LA causes less effects on immune function than OA in Chronic appendicitis postoperatively （P 〈0.01）.
