仪器法检测ESBLs“灰色区间”的临床处置

摘要 超广谱β内酰胺酶(Extended—spectrum β—lactamases ESBLs)是一类由细菌质粒介导,能水锯头孢噻肟(CTX)、头孢他啶(CAZ)等亚氨基β-内酰胺类及氨曲南等单环酰胺类抗生素,并可被克拉维酸(CLA)等β-内酰胺酶抑制剂所抑制,在临床上易造成医院内感染、暴发流行。准确、及时地诊断ESBLs非常重要。近年来,随着自动化微生物仪应用的增多,仪器法虽然具有特异性好,但灵敏度低,特别是当产酶量恰好落在3~4个稀释度、抑菌圈直径差在5mm时成为仪器检测的“灰色区间”,易造成漏检。
