摘要
目的了解目前云南农村卫生医疗机构存在的问题并探讨应对策略.方法以我省最早的20个新型农村合作医疗试点县之一弥渡县为样本,按距离县城近、中、远3个层次选择太花、寅街、牛街3个乡卫生院进行调查.结果(1)3个乡卫生院各有1名管理人员(院长),3个乡每千人口的卫生技术人员和床位数分别为0.45和0.53;(2)卫生技术人员中,医学中专、大专、本科及以上学历者分别占77.65、6.7%和3.4%,具有主治医师及以上职称者仅11人;(3)各卫生院均消灭危房,就医环境明显改善;(4)各卫生院基本设备数量少,种类不全,且利用率低;(5)各卫生院收入以药品收入为主,支出以药品和人头费为多.结论农村卫生资源匮乏,经营状况不乐观.建议通过多渠道提高在岗卫技人员学历,选用优秀管理人才,更新观念,创新服务模式.
Objective To find explore response strategies. Method out the problems in the face of the rural commune hospital in Yunnan, and Tai-hua, Yin-jie, Niu-jie commune hospitals range near, medium, far from Mi Du County town were surveyed. Results (1) There was only one manger (principle) in each hospital. One thousand of population in the 3 communes had 0.45 medical personnel and 0.53 sickbeds. (2) Among medical personnel, 77.6% have been trained in secondary vocational school and 6.7% in medical college and 3.4% in medical university or above. There were only 11 attending doctors. (3) All three hospitals had no dangerous building. The environment of treatment was great improved. (4) The basic apparatus were old and a few, and utilizable efficacious of them was very low. (5) The main income was from medicines, and main expenditure was for medicines and personnel. CoNclusioNs The health resource in rural commune are not enough, and hospital situation of running is not optimistic. Suggestion: to enhance the record of formal schooling, for medical personnel to select excellent manager, renew view, and create more medical service patterns.
