摘要
目的:探讨胸廓内动脉的血管解剖及其在介入治疗中的价值。方法:在22例肝癌及胸部疾患的介入治疗中运用了经胸廓内动脉插管DSA造影,对胸廓内动脉参与病变供血的病例进行了介入治疗,并进行了经验总结。结果:胸廓内动脉参与病变供血12例,胸廓内动脉行程长、分支及供血器官较多,对胸廓内动脉参与供血的病变进行介入治疗有重要价值。结论:胸廓内动脉在肝癌及胸部疾患的介入治疗中不应忽视。
Objective:To study the anatomy of internal thoracic artery (ITA) and its importance for interventional treatment. Methods: In the interventional treatment of liver cancer and thoracic diseases, catheterization and digital subtraction angiography (DSA) of the ITA was performed in 22 cases. In 12 cases it was shown that the lesions were partly supplied by this artery and transarterial embolization or infusion via this artery was performed. The therapeutic effect was analysed. Results:The internal thoracic arteries displayed by DSA had a long course and many branches and supplied many or gans. In four patients with liver cancer, the lesion diminished in size and/or had imcreased lipiodol retention after TACE via the ITA. In three cases with lung cancer, the tumor size was reduced after infusion chemotherapy via bronchial artery and ITA. In four patients with severe hemoptysis, the bleeding was stopped in 72h after embolization of bronchial arterry and ITA using PVR granules and gelfoam. In one ease of breast cancer treated with infusion chemotherapy via ITA before resec tion, pathological examination comfirmed necrosis of the cancer cells. Conclusion : Internal thoracic artery may participate the blood supply of the hepatic and thoracic lesions. ITA should not be neglected for the interventional treatment of lesions of liver and chest. For lesions also supplied by ITA,it has great value to perform interventional treatment via this artery.
