CT引导经皮穿刺活检在肺部肿块诊断中的诊断价值与安全性

Therapeutic value and safety of CT-guided transthoracic needle biopsy for diagnosis of suspected pulmonary nod-ules Huadong Hospital Affiliated to Fudan University

目的探讨应用Bard活检针在CT引导下肺穿刺活检的诊断准确性和安全性。方法对痰细胞学阴性,纤支镜不能确诊的36例肺部外周性病变患者在肺部CT扫描定位下,应用Bard活检针经皮肺穿刺活检取材,行病理学诊断。结果36例病人穿刺36次,阳性33例(91.6%).阴性3例(8.4%)。总检出率91.6%,其中恶性病变率为86.1%,术后少量咯血2例。结论应用Bard活检针在CT引导下经皮肺活检是一种安全、有效的检查手段,对肺周围性病变具有较高的诊断价值。

Purpose To validate the therapeutic effect and safety of CT-guided transthoracic needle biopsy for the diagnosis of pulmonary nodules. Patients and Methods Included in the present study were 36 patients with peripheral pulmonary nodules whose nature was not confirmed by sputum cytology or fibroscopy. Core needle biopsy was done for pathologic study by using Bard Innovations Magnum Reusable Biopsy System under CT guidance. Results A total of 36 biopsies were performed for the 36 patients. The result was positive in 33 patients （91.6%）, and negative in the remaining three patients （8.4 %）, the overall detection rate being 91.6%, including 86.1% of patients with malignancies. There were two patients who experienced mild hemoptysis after the biopsy procedure. Conclusion CT-guided transthoracic needle biopsy using Bard Reusable Biopsy System proved to be safe and effective for the diagnosis of suspected peripheral pulmonary nodules.
