106例意义不明确的宫颈不典型鳞状上皮细胞的病理诊断

The Pathologic Diagnosis of 106 Cases with Atypical Squamous Cells of Undetermined Significance(ASCUS) in Cervix

[目的]探讨意义不明确的宫颈不典型鳞状上皮细胞(atypical squamous cells of unde-termined significance,ASCUS)的病理诊断。[方法]对106例细胞学诊断为ASCUS的患者行HPVDNA检测,同时在阴道镜下多点活检。[结果]106例ASCUS患者中慢性炎症81例(76.42%),其中高危型HPV感染20例,占24.69%(20/81);宫颈上皮内瘤变(CIN)25例(CINⅠ12例,CINⅡ9例,CINⅢ4例),其中高危型HPV感染23例,占92.0%(23/25);慢性炎症和CIN患者的HPV感染阳性率有显著性差异(P<0.05),且两型及以上HPV感染阳性率两组也有显著性差异(P<0.05)。[结论]ASCUS患者行阴道镜下活检结合HPV检测,能进一步明确诊断。

[Purpose] To investigate the pathologic diagnosis of cervical atypical squamous cells of undetermined significance（ASCUS）. [Methods] One hundred and six cases with ASCUS underwent human papilloma virus（HPV） DNA test and colposcopic muhiple punch biopsies. [Result] Among 106 cases with ASCUS, 81 cases were pathologically diagnosed as chronic inflammation （76.42%）, with high risk type of HPV infection in 20 cases（24.69%）; 25 cases were pathologically diagnosed as cervical intraepithelial neoplasia （CIN） （CIN Ⅰ , 12 cases; CIN Ⅱ, 9 cases; CIN Ⅲ, 4 cases）, with high risk type of HPV infection in 23 cases （92.00%）. There was significant difference in the positive rate of high risk type of HPV infection between chronic inflammation and CIN （P〈0.05）. And also the infection rate with 2 or more types HPV was significant difference between the two groups（P〈0.05）. [Conclusion] Colposcopic multiple punch biopsies combined with HPV detection contribute to the final diagnosis of ASCUS .
