旋转型痉挛性斜颈的三联手术治疗

Surgical Treatment for the Rotation Type Spasmodic Torticollis

目的探讨旋转型痉挛性斜颈(spasmodic torticollis,ST)的手术治疗方法及疗效。方法对215例旋转型ST的不同术式进行分析。结果全组215例中斜颈症状消失150例(69.8%);显效43例(20.0%);进步18例(8.4%);无效4例(1.8%)。无1例死亡、残疾。结论颈部痉挛肌肉选择性切除术、颈神经后支选择性切断术、副神经切断术(三联术)是治疗旋转型ST安全有效的方法。

Objective The research was directed to the exploration of the surgical methods and techniques to cure the rotation type ST, as well as the operation results. Methods 215 cases from 1994 to 2007 were collected and clinical materials were retrospectively analyzed about the clinical classification, selection of the surgical methods, anatomy of ST, pathologic changes of the nerves and the examination Results such as EEG in the cervical muscles, CT in the neck and head, and MRI in the head. Results The evaluation of clinical effects over 6 months post-operation： Among the 215 cases, 150 were recovered（69.8%）, 43 were markedly effective（20.2%）, 18 made progress（8.4%）, with 4 cases of inefficiency（1.8%）. Conclusion The surgical treatment of accessory neurotomy, selective resection of cervical spasmodic muscles and selective neurotomy of cervical for the rotation type ST is safe and effective.
