Role of insulin resistance in the development of nonalcoholic fatty liver disease

摘要 目的探讨胰岛素抵抗(IR)在非酒精性脂肪性肝病(NAFLD)发病中的作用。方法将18只Wistar大鼠按随机区组设计分为正常对照组(normal control,NC组)、饱和脂肪酸组(saturated fatty acid,SFA组)、不饱和脂肪酸组(unsaturated fatty acid,USFA组)。观察空腹血糖(FPG)、空腹胰岛素(FINS)、血浆游离脂肪酸(FFA)、转氨酶(ALT)、肝匀浆肿瘤坏死因子-α(TNF-α)、丙二醛(MDA)、甘油三酯(TG)及肝脂肪变程度,采用放射免疫法测定TNF-α、FINS,计算胰岛素抵抗指数(IRI)。结果SFA组和USFA组血浆FFA、IRI、肝匀浆TNF-α、MDA、TG与正常对照组比较均明显增高,有统计学差异;与正常组比较,饱和组ALT显著增高,有统计学差异。三组血糖无统计学差异。结论IR在NAFLD形成过程中发挥重要作用。 Objective To explore the effect of insulin resistance on nonalcoholic fatty liver disease. Methods Eighteen rats were randomly divided into three groups:normal control group(NC group),saturated fatty acid group(SFA group),unsaturated fatty acid group(USFA group).The levels of fasting plasma glucose(FPG),fasting insulin(FINS),free fatty acid(FFA) and alanine aminotransferase(ALT)in plasma,and tumor necrosis factor-α(TNF-α),malondialdehyde(MDA),triglyceride(TG)in liver homogenate,and the changes of fatty liver were detected.TNF-α and FINS were examined with radioimmunoassay.Insulin resistance index(IRI)was calculated. Results The levels of plasma FFA,IRI,liver homogenate TNF-α,MDA and TG in SFA and USFA groups were signi-ficantly higher than in NC group.The level of ALT in SFA group was significantly higher than in NC group.The level of FBG had no statistical difference between three groups. Conclusion IR may play an important role in the nonalcoholic fatty liver disease.
