脂联素基因276位点多态性与2型糖尿病肾病的关系 被引量:7

摘要 目的研究山西地区汉族人脂联素基因276位点单核苷酸多态性(SNP)与2型糖尿病肾病之间的关系。方法选取262名山西地区汉族人,包括正常对照76例,2型糖尿病无肾病患者108例,2型糖尿病肾病患者78例。观测所选人群的临床指标,并针对脂联素基因276位点用聚合酶链反应-限制性内切酶长度多态性(PCR-RFLP)技术进行基因分型。结果①SNP276基因型及等位基因频率在正常对照组和2型糖尿病组之间分布差异有统计学意义(P<0.05),2型糖尿病组G等位基因频率显著高于正常对照组(P<0.01)。SNP276基因型及等位基因频率在2型糖尿病无肾病组与2型糖尿病肾病组之间分布差异无统计学意义(P>0.05)。②正常对照组GG或GT型体重指数、收缩压、空腹胰岛素、胰岛素抵抗指数、空腹血糖均显著高于TT型。2型糖尿病无肾病组GG或GT型收缩压、空腹胰岛素、胰岛素抵抗指数、空腹血糖均显著高于TT型。2型糖尿病肾病组GG或GT型收缩压、胰岛素抵抗指数显著高于TT型。结论在中国山西汉族人群中,与TT型相比,携带GG+GT型者患2型糖尿病的危险因素增加。G等位基因是胰岛素抵抗、2型糖尿病的危险因素;脂联素SNP276G/T与2型糖尿病相关,与糖尿病肾病无关。 Objective To investigate the relationship between single nucleotide polymorphism (SNP) at position 276 in adiponectin gene and type 2 diabetic nephropathy in Han ethnicity in Shanxi province in China. Methods SNP276 of adiponectin gene was genotyped in 76 normal controls, 108 type 2 diabetic patients without diabetic nephropathy(DN) and 78 type 2 diabetic patients with DN by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Their clinical parameters were examined. Results ①There was significant difference in the genotypes and allele frequency distributions of SNP276 between type 2 diabetes mellitus(T2DM)patients and controls(P〈0.05).The frequencies of G allele in T2DM patients were higher than that in normal controls(P〈0.01).The distribution of genotypes and allele frequencies of SNP276 polymorphism had no statistical difference be-tween T2DM patients with DN and T2DM patients without DN(P〉0.05).②The body mass index(BMI),systolic blood pressure, fasting plasma insulin,HOMA-IR and fasting plasma glucose were higher in GG or GT genotype than in TT genotype in normal controls.The systolic blood pressure,fasting plasma insulin,HOMA-IR and fasting plasma glucose were higher in GG or GT genotype than in TT genotype in T2DM patients without DN.The systolic blood pressure,HOMA-IR was higher in GG or GT genotype than in TT genotype in T2DM patients with DN. Conclusion The risk of T2DM increases in GG or GT genotype compared to TT genotype.SNP276 polymorphism of adiponectin gene is associated with type 2 diabetes in the population of Han ethnicity in Shanxi province,but not with DN.The G allele may be a risk factor of insulin resistance and type 2 diabetes in Han ethnicity of China.
