摘要
目的观察不同部位的颅骨震动对健康人体前庭诱发肌源性电位的潜伏期及幅度的影响,以明确颅骨震动对双侧球囊输入的影响,从而为颅骨局部震动可刺激球囊提供初步证据。方法16名健康志愿者,以随机次序,分别在4次不同颅骨震动条件下(无颅骨震动、左乳突震动、右乳突震动、前额正中震动),在双侧胸锁乳突肌上记录双耳强短声刺激诱发的前庭诱发肌源性电位。对4种颅骨震动条件下记录的前庭诱发肌源性电位的潜伏期及双侧幅度比进行比较。结果同无震动时相比,震动不同颅骨部位对前庭诱发肌源性电位的潜伏期未有明显影响(F=0.000,P〉0.05),左侧乳突震动可增加前庭诱发肌源性电位双侧幅度比(L/R)(F=15.148,P〈0.05),右侧乳突震动可降低前庭诱发肌源性电位双侧幅度比(F=4.257,P〈0.05),但前额正中震动对前庭诱发肌源性电位双侧幅度比未有明显影响(F=0.182,P〉0.05)。结论颅骨震动可刺激球囊,乳突震动主要增加同侧球囊输入,而前额正中震动对双侧球囊输入产生相同的影响。
Objective To investigate the effect of different location skull vibration on vestibular-evoked myogenic potential(VEMP) in healthy humans and to identify the influence of skull vibration on bilateral saccule input and so as to provide preliminary evidence that topical skull vibration can stimulate saccule. Methods Sixteen healthy volunteers served as subjects. Loud click evoked VEMPs were recorded in bilateral sternocleidomastoid muscles (SCMs) under four skull vibration conditions(no skull vibration, left mastoid vibration, right mastoid vibration and forehead midline vibration) in random order. Latencies and bilateral amplitude ratio(L/R) of VEMP recorded under four conditions were compared. Results Compared with no skull vibration, different location skull vibration have no effect on the latencies of VEMP (F =0. 000, P〉 0. 05), however left mastoid vibration can increase bilateral amplitude ratio (L/R) of VEMP (F = 15. 148, P 〈 0. 05) and right mastoid vibration can decrease bilateral amplitude ratio (L/R) of VEMP ( F = 4. 257, P 〈 0. 01 ), otherwise forehead vibration have no effect on bilateral amplitude ratio(L/R) of VEMP(F= 0. 182, P〉0.05). Conclusion Skull vibration can stimulate saccule and mastoid vibration augment mostly ipsilateral saccule input but forehead midline vibration have same impact on bilateral saccule input.
