腹腔镜胆囊切除术腹腔引流的应用及术后管理

目的探讨腹腔镜胆囊切除术(LC)后,腹腔引流的应用及管理体会。方法总结腹腔镜胆囊切除术后146例引流的应用。结果腹腔镜胆囊切除术1600例,行腹腔引流146例,引流管均置于肝下小网膜孔处。无异常情况则于24—48小时拔除。其中3例术后并发胆瘘,经治疗后治愈,1例肝下积液形成,经抗感染、中药治愈。其余病例均无并发症,全组无死亡率。结论腹腔镜胆囊切除术是一项新的治疗手段.对术后引流的管理提出了新的要求。正确的处理对手术的成功占十分重要的作用。本组146例,在引流放置的技巧,指征的选择及管理方面均取得良好效果。明显降低了并发症的发生,具有安全简便、创伤小、便于临床观察与护理的特点。
