应用国产封堵器介入治疗左向右分流型先天性心脏病的疗效评价

The Effective Evaluation of Treatment for Congenital Heart Disease Using Interventional Therapeutic Procedures from Left to Right Shunt

目的探讨应用国产封堵器治疗左向右分流型先天性心脏病的效果及安全性。方法368例患者,其中动脉导管未闭(PDA)94例,房间隔缺损(ASD)123例,室间隔缺损(VSD)148例,复合型先天性心脏病(PDA+ASD)3例。均在X线指引和经胸超声心动图监视下按Amplatzer方法经导管置入封堵器。结果全组成功率95.4%,术后即刻及随访观察均无残余分流,达到完全封堵治疗目的;无主要并发症发生。结论经导管置入封堵器治疗PDA、ASD和VSD是一种操作较简便、成功率高、疗效可靠及安全性良好的介入治疗方法。

Objective To search for and evaluate the effect and safety which applied to the complications of transcatheter closure of congenital heart disease （CHD） with left to right shunt using the domestic-made occluder. Methods Total of the 368 patients, there were 94 cases of patent ductus arteriosus （PDA）, 123 cases of atrial septal defects （ASD）, 148 cases of ventricular septal defects （VSD）, and 3 cases of PDA＋ASD. According to the methodology described by Amplatzer, all the patient were interventional treated with the occluder through the percutaneous procedure under fluoroscopy angiography and transthoracic echocardiography （TTE）. Results The success rate of all groups was 95. 4%. No residual shunts were found after the procedure and during follow--up of 1 month, 3 months. No major complications occurred in this group. Conclusion Transcatheter of PDA, ASD and VSD with the Amplatzer occluder is a simple and effective method with a high success rate and good occlusion effect.
