小切口动力加压髋螺钉内固定治疗老年股骨转子间骨折

目的探讨小切口动力加压髋螺钉(DHS)内固定治疗老年股骨转子间骨折的可行性及疗效分析。方法对35例65岁以上老年人股骨转子间骨折,施行了DHS内固定术,记录手术切口长度、手术时间、术中出血量及术后引流量。结果手术切口7～9cm(平均7.7cm)。35例获随访,随访0.5年～4.17年,平均随访1.93年,按美国Harris标准进行临床效果评定,优21例、良11例、差3例,优良率达91.4%。结论小切口DHS内固定术是治疗老年人股骨转子间骨折的理想方法。

Objective Debate on feasibility of the intertrochanteric fracture therapy in the elderly patients using micro-incision internal fixation with dynamic hip screw（DHS） and corresponding analysis of therapeutic effects. Methods 35 elderly patients （≥65 years） with intertrochanteric fracture have been treated by the DHS internal fixation,and enregistered the length of incision,the time of operation, the gross of heamorrhage in operation,and the gross of drainage after operation. Results The length of incision operation is 7 to 9 centimeters （an average of 7.7 centimeters）. 35 patients were followed up for 0.5 to 4.17 years ,with an average of 1.93 years, Harris Score System was uesed to evaluate the function of articulationes coxae,and 21 cases were excellent, ll cases were fine,3 cases were dissatisfacory.The satisfaction rate of the treatment was 91.4%. Conclusion The micro-incision internal fixation with DHS is a reliable method in treatment of intertrochanteric fracture in the elderly patients.
