摘要
目的探讨皮肤恶性黑色素瘤的临床病理特征和鉴别诊断。方法对29例经病理诊断为皮肤恶性黑色素瘤的病例进行临床病理特征回顾性分析。结果恶性黑色素瘤细胞形态多样,分为上皮样细胞、棱形细胞、小痣样细胞、透明细胞及瘤巨细胞等,排列也多样化,呈巢状、梁状、腺泡状、束状、编织状漩涡状或弥漫分布。免疫组化染色显示瘤细胞S-100蛋白、HMB45、Vimentin阳性率分别为100%、93%、93%。结论恶性黑色素瘤的组织结构和病理形态呈多样性,其诊断和鉴别诊断主要靠病理组织学和免疫组织化学。
Objective To investigate chnicopathological characteristics and differential diagnosis of maligrant melanoma of the skin. Method The histopathological characteristics and clinical date of 29 cases were analyzed retrospectively. Results By microscopy, the ceils type were composed of epithelioid, spindle, nevus like, hylocyte and tumour giant. The tumour cells were arranged in nest-like, beamlike, adeniform fascicle-like, laciform, whirlpool and diffusion pattern. The immunohistochemicaily, the positive rates of S-100 protein, HMB45, Vimentin in 29 cases were 100%, 93%, 93%. Conclusion The histopathological structures and morphological characters of malignant melanoma are complex. The diagnosis relies mainly on histopathological and immunohistochemically findings.
