双侧唇裂或唇腭裂Ⅰ期修复后不同口哨畸形修复术式的选择研究

Selection of different operative procedures for different grades of whistling deformity resulting from primary repair of bilateral cleft lip or cleft lip and cleft palate

目的评价双侧唇裂或唇腭裂Ⅰ期修复后不同口哨畸形的修复效果。方法选我院治疗的双侧唇裂或唇腭裂术后上唇口哨畸形患者136例为研究对象,将136例患者按上颌门齿牙冠及相应牙龈暴露的程度,将口哨畸形分为3级。Ⅰ级采用双蒂推进红唇肌瓣修复法;Ⅱ级使用改良Abb啨瓣修复法;Ⅲ级主要使用Abb啨瓣修复法?峁∈鹾笏娣?个月至15年,上唇解剖结构基本恢复。结论按不同分级选择不同修复方法,是一种具有临床指导意义的科学方法,可被推广应用。

Objective To evaluate the effects of repair using different operations for different whistling deformities which resulted from the primary repair of bilateral cleft lip or cleft lip and cleft palate. Methods According to different degrees of exposure of the crowns and gums of the anterior incisors, 136 patients with whistling deformities which resulted from primary repair of bilateral cleft lip or cleft lip and cleft palate, were classified into three grades. According to the assigned grade, different operative techniques were used for following as： Double movable mucomuscular complex flaps were used for grade Ⅰ deformities; the modified Abbe flap was used for repair of grade Ⅱ; and for grade m whistling deformity, used mainly the Abbe flap. Results Patients were followed Fostoperatively from six months to fifteen years, and the anatomical structures of the upper lip were nearly normal after recovery. Conclusion Choise of different repair methods for whistling deformity based upon grade of the deformity is a scientific approach which has important clinical value.
