手术治疗踝关节骨折

Treating ankle joint fracture by simplified surgical techniques

目的探讨手术治疗踝关节骨折的疗效。方法63例踝关节骨折患者随机分为2组,采用可吸收螺钉内固定治疗38例,金属内固定治疗25例。结果63例随访2个月-2年9个月。疗效根据Kofoed评分标准进行评定：可吸收螺钉组：优14例,良18例,及格5例,差1例;金属内固定组：优15例,良6例,及格3例,差1例。总优良率为84.1%。两组均未发生感染、骨不连、骨折畸形愈合等并发症,金属内固定组中1例发生下胫腓固定螺钉断裂。结论手术治疗不同类型踝关节骨折,方法简单,效果可靠。

Objective To overview the results of surgical treatment the ankle joint fracture. Methods 63 cases of consecutive patients with ankle joint fractures were randomly divided two groups, of which 38 were treated with absorbable screws, 25 with anatomy metal plate. Results The follow-up period varied from 2 months to 33 months. All patients were assessed with Kofoed＇s ankle scoring system. In the absorbable screws group, the clinical results were excellent in 14, good in 18, fair in 5, and poor in 1. In the anatomy metal plate group, the clinical results were excellent in 15, good in 6, fair in 3, and poor in 1. The total rate of excellent and good results was 84. 1%. There was no local complication, malunion or nonunion of the fractures and deformity of the ankle. However, in the anatomy metal plate group, the inserted screw to distal tibiofibular syndesmosis was broken in 1 case. Conclusions It is simple and reliable to treat various kinds of ankle joint fractures by the surgical techniques.
