直尺辅助下阴茎体近端皮肤等腰梯形切除术治疗包皮过长

Ruler-assisted isosceles trapezoid resection of skin of proximal shaft of penis for redundant prepuce

目的：介绍直尺辅助下阴茎体近端皮肤等腰梯形切除术治疗包皮过长的优点。方法：2000年11月～2007年4月对67例包皮过长患者行直尺辅助下阴茎体近端皮肤等腰梯形切除术。本组患者年龄19～43岁,平均26岁。本式中直尺的辅助作用体现在：①测量近端切口与远端切口的长度,使两者相等;②沿直尺切开近端切口与远端切口。结果：直尺的辅助作用使阴茎体近端皮肤切开容易,切口整齐;容易保证近端切口与远端切口长度相等。等腰梯形切除保证切口足够大,使术后切口无狭窄;使阴茎背侧皮肤切除略多,符合勃起的阴茎对皮肤的自然要求。本组手术时间22～50分钟,平均35分钟,出血量3～10ml。随访2周～6年,术后切口无感染,少量渗血12例,切口背侧裂开1.0～3.0cm4例。愈合后切口痕迹轻微、隐蔽,切口无狭窄,系带完整性未破坏,系带形态美学结构仍存在。结论：直尺辅助下阴茎体近端皮肤梯形切除术治疗包皮过长操作容易,切口愈合后阴茎外观自然。

Objectives： To evaluate the advantages of ruler - assisted isosceles trapezoid resection of skin of proximal shaft of penis for redundant prepuce, Methods： From November 2000 to April 2007, 67 patients with redundant prepuce, who were 19 to 43 years old with average 26 years, were performed ruler - assisted isosceles trapezoid resection of skin of proximal shaft of penis, The riders had two functions in the resection operation： （ 1 ） To assist in measuring length of proximal and distal incisions, and to insure the equality of two incisions; （2） To assist in cutting of the two incisions. Results： The two incisions of skin of proximal shaft of penis, the proximal and the distal, were cut conveniently, neatly and equally with the aid of the rulers, The isosceles trapezoid resection made the incisions long enough not to be narrow and the length of dorsal penile skin of shaft was a bit longer than the one of ventral penile skin which is adapted to the natural demand of erectile penis. The mean operation time was 35minutes （22 - 50 min） and the volume of blood loss were 3 to 10ml. The 2 weeks to 6 years follow - up indicated that incisions were healed beautifully with no infection and 12 cases haemorrhaged slightly, 4 cases disruptive from 1.0 to 3.0 era. The incision scars were unconspicuous and concealed; and the narrow phenomenon was not found. The frenulum integrity wasn＇t destroyed and its aesthetic value to the whole penis was still preserved. Conclusion： The ruler - assisted isosceles trapezoid resection of skin of proximal shaft of penis for redundant prepuce is performed conveniently and the postoperative outlook of penis is natural.
