直肠癌腹部手术切口改进及临床意义

The Improvement and Clinical Significances of the Abdomenal Incision in Rectal Cancer

报道１１２例直肠癌采用右下腹旁正中切口手术，同期行胆囊切除及胆道探查８例，肝转移灶切除１１例，肝动脉插管栓化疗６例，门静脉插管化疗２１例。作者认为右侧切口不影响直肠癌术野暴露及操作，可减少切口受造瘘口粪便污染，减少切口疝及造瘘口旁疝发生，有利于处理肝、胆并存病变，值得推广采用。

112 cases of rectal cancer operation with lower right paramedian incision were reported in this paper.Simultaneously,8 cases were taken cholecystectomy or exploration of common bile duct,and 4 cases were resected the lession of hepatic metastasis,catherter embolism and chemotherapy from hepatic artery in 6 cases.Authors suggest that the right lower paramedian incisiion dose not hinder the explorating and operating,and it can reduce the feces contamination from artificial anus,lower down the rate of incision hernia and the parahernia of fistulation.Furthermore,it is easy to treat the complicated hepatic and cholectic lession.So,it is worth to spread and use.
