磷脂酶A_2和弹力蛋白酶活性与坏死性胰腺炎并发肺损伤的关系

摘要 ①目的探讨磷脂酶A2(PLA2)和弹力蛋白酶活性与坏死性胰腺炎并发肺损伤的关系。②方法复制动物模型,定量测定对照组和实验组大鼠的磷脂酶A2和弹力蛋白酶活性。③结果PLA2活性只在疾病早期显著升高(q=5.087,P<0.05);其中6,12h两时间,实验组的弹力蛋白酶的活性皆高于对照组,两组比较差异有显著性(q=8.289,4.276,P<0.01,0.05),且与肺内粒细胞浸润数量呈正相关(r=0.964,P<0.01)。④结论PLA2在肺损伤中是一个早期效应因子,而弹力蛋白酶则是一个持续损害因子,其先后导致急性胰原性肺气肿及成人呼吸窘迫综合征。 Objective To study the relationship between phospholipase A 2(PLA 2) activity and elastase activity and acute necrotic pancreatitis complicated with pulmonary damage. Methods A model of active necrotic pancreatitis in rat was established. PLA 2 activity and elastase activity of control and expermental groups were determined. Results PLA 2 activity in the experiment group was significantly higher than that in the control group at early stage( q=5.087, P <0.05). The elastase activity in the experimental groups at 6h, 12h were significantly higher than that of the control group( q=8.289, 4.276, P <0.01, 0. 05) . The elastase activity was positively correlated with the count of infiltrated neutrophils in the lung( r=0.964,P<0 01 ). Conclusion PLA 2 is an early injury factor in pancreatitis pulmonary damage. Elastase is a continuous injury factor in the whole course and can lead to acute pancreatic pulmonary emphesema and ARDS with the development of the disease.
