经皮动脉导管溶栓治疗急性肢体动脉栓塞11例报告

Percutaneous transcatheter thrombolysis for acute thromboembolic arterial occlusions of extremities:a report of 11 cases

目的探讨经皮动脉导管溶栓治疗急性肢体动脉栓塞的可行性及疗效。方法对11例经皮动脉导管溶栓治疗的急性肢体动脉栓塞患者的临床资料进行总结分析。结果11例患者中10例(91%)溶栓治疗成功,血管造影显示血流通畅,临床症状完全缓解;1例无效者血管造影显示与治疗前无变化,改行手术治疗。1例在溶栓过程中发生远端血管出血,采取局部压迫止血,其余患者未发生并发症。无截肢和死亡者。结论经皮动脉留置导管溶栓治疗急性动脉栓塞是一种安全、疗效优良的方法。

Objective To explore the feasibility and effect of transcatheter thrombelysis for acute thromboembolic arterial occlusions of extremities. Methods From October 2005 to October 2007, the results treated with catheter-directed thrombolysis for acute arterial thromboembelia of extremities were evaluated retrospectively. Results Thrombelytic success was achieved in 10 of 11 cases （91%）. Angiographies of these 10 patients showed complete patency of the suffered arteries, and clinical symptoms were completely relieved. In the patient failed with thrombolytic, there was no change in clot burden and in clinical improvement, befor and after treatment which required further surgical revascularization. Remote bleeding was seen in one patient during the course of thrombolysis and was stopped by loeal compression. No other complications occurred. There was no case of amputation or death. Conclusions Transcatheter thrombelysis is a safe and effective treatment for patients with acute thromboembolic arterial occlusions of extremities.
