实时荧光定量PCR检测BAALC基因在正常核型急性髓系白血病中的表达 被引量:6

Clinical significance of real-time quantitative RT-PCR for detection of BAALC in patients with normal-cytogenetics acute myeloid (NC-AML)

摘要 目的探讨BAALC基因在NC-AML中的表达及其临床意义。方法运用实时荧光定量RT-PCR方法检测BAALC基因在48例NC-AML患者中的表达。结果以48例NC-AML患者骨髓BAALC基因中位表达为界,分为高表达(n=26)和低表达(n=22)组。BAALC高表达组与低表达组相比,患者的年龄、性别、血红蛋白量、血小板计数、外周血及骨髓中原始细胞比例差异无显著性;BAALC高表达组外周血高白细胞、FAB分型中M0和M1及CD34表达率较低表达组高;BAALC高表达组完全缓解率(CR)58%较低表达组CR87%差异有显著性(P<0.05);BAALC高表达组复发率42%较低表达组复发率19%差异有显著性(P<0.05)。结论BAALC基因高表达是NC-AML患者预后不良的分子标志。 [Objective] To evaluate the expression of BAALC gene in NC-AML patients and its clinical implication. [Methods] BAALC expression level in BM of 48 NC-AML patients were detected by real-time RT-PCR through LightCycler. [Results] Patients were dichotomized at BAALC's median expression into low (n=22)and high (n=26)expressers. There were no significant differences between high BAALC and low BAALC expressing patients with respect to age, sex, hemoglobin, platelet count, percentage of blasts in bone marrow or blood. High BAALC expression was associated with higher white blood count, the more immature FAB subtypes M0/Mll (P〈0.05), and the higher CD34 expression; high BAALC expressers showed significantly inferior achieved CR than low BAALC expressers (58% v 87%; P 〈0.05); high BAALC patients relapsed more frequently (42% v 19%; P 〈0.05). [Conclusion] High BAALC expression predicts an adverse prognosis and may define an important risk factor in AML with normal cytogenetics.
