海绵窦颈内动脉损伤所致严重鼻出血的血管内栓塞治疗(附2例报告)

ENDOVASCULAR EMBOLIZATION OF SEVERE RHINORRHAGIA RESULTING FROM CAROTID-CAVERNOUS INJURY

报告海绵窦颈内动脉损伤引起严重鼻出血２例，其中１例为突入蝶窦的巨大假性动脉瘤，１例为已进行过颈总动脉结扎和开颅颈内动脉夹闭术的颈内动脉海绵窦瘘，分别采用经股动脉行颈内动脉栓塞和经眼静脉行颈内动脉海绵窦瘘栓塞治疗，取得满意效果。

Two cases of severe rhinorrhagia resulting from carotid cavernous injury were reported. One of them suffered from a tramatic giant pseudoaneurysm herniating into sphenoid sinus and was treated successfully by transarterial balloon embolization. The other was caused by a tramatic complex internal carotid cavernous fistula which was not treatable by combined extracranial ligation of the carotid artery and intracranial clipping of internal carotid artery but was treated effectively by transvenous coil and dry dural strip embolization of the fistula through the superior ophthalmic vein. It is suggested that endovascular embolization is the first choise treatment of rhinorrhagia for carotid-cavernous injury.
