脐血流监护仪诊断胎儿脐带绕颈临床价值

Clinical Value of Umbilical Blood Flow Monitoring for Diagnosing Fetues Umbilical Cord Around Neck(UCAN)

目的评价脐血流仪诊断胎儿脐带绕颈临床价值，探讨脐带绕颈与胎儿宫内缺氧的关系。方法选择孕37-42周单胎头位孕妇进行脐血流仪检查，并检测出脐带绕颈、测定脐动脉收缩期峰值速度（S）、舒张末期血流速度（D），计算S／D值。结果脐血流仪诊断脐带绕颈100例孕妇中，分娩后证实92例。其中S／D〉3．0有11例；S／D2．5-3．0有18例；S／D〈2．5有63例。结论脐血流仪不仅可以诊断胎儿脐带绕颈，更可通过测定脐动脉血液动力学指标提示胎儿宫内危险状况，为产科临床及时有效的处理提供科学依据。

Objective To investigate the clinical value of cord blood flow monitoring for diagnosing UCAN and discuss the relationship between fetal intrauterine hypoxia and UCAN. Methods The pregnant women （37-42 gestational weeks） were examined by Cord Blood Flow monitoring, UCAN was diagnosed and umbilical artery peak systolic velocity（S）, end-diastolic velocity （D） were measured, the ratio of S/D was calculated. Results 100 pregnant women were diagnosed as UCAN by cord blood flow monitoring,92 were confirmed after delivery. Conclusions Cord blood flow monitoring can not only be used to diagnose UCAN but also show the fetal position inside uterus,it provides a clinical scientific basis for treatment.
