腋下小切口与电视胸腔镜手术治疗自发性气胸的比较

Comparison of Video-assisted Thoracoscopic Surgery and Limited Axillary Thoracotomy for Spontaneous Pneumothorax.

目的：比较腋下小切口与电视胸腔镜手术治疗自发性气胸的疗效。方法：2005年7月-2007年7月对本院自发性气胸患者160例分别应用于腋下小切口与电视胸腔镜手术治疗自发性气胸（单侧手术）。比较2组手术时间、术中出血量、术后胸腔引流管留置时间。结果：腋下小切口组手术时间为60.3±11.2分钟,胸腔镜组出血量为50.2±7.8分钟（P〈0.05）,有统计学差异;小切口组出血量为51.6±5.3毫升,胸腔镜组出血量为42.5±3.6毫升（P〈0.05）,有统计学差异;术后胸引管留置时间小切口组8.7±2.3天,胸腔镜组5.2±1.8天（P〈0.05）,有统计学差异。结论：电视胸腔镜手术治疗自发性气胸较之腋下小切口手术疗效确切,创伤小。

Objective： To compare the outcomes of video-assisted thoracosopic surgery （VATS） and limited axillary thoracotomy （LAT） in the treatment of spontaneous pneumothorax, Methods ：160 patients with spontaneous pneumothorax from July, 2005 and July, 2007 in Siehuan Provincial People＇s Hospital were included, LAT was performed in 95 cases and VATS was performed in 65 eases, The operating time, blood loss during operation and chest drainage time were compared between the two groups. Results：The operating times in LAT group and VATS group were 60.3 ±11. 2 min and 50.2 ± 7.8 min （ P 〈 0.05 ） . respectively ; the blood loss amounts during operation were 51.6 ± 5.3 ml and 42.5 ± 3.6 ml（ P 〈 0.05）, respectively; the drainage times were 8.7 ± 2.3 d and 5.2 ± 1.8 d, （ P 〈 0.05 ）, respectively. Conclusion ： VATS is effective and minimally invasive for treating spontaneous pneumothorax.
