硫酸镁甘油湿敷在嵌顿痔水肿防治中的应用

Application of wet dressing with glycerine magnesium sulfate on preventing and treating oedema of incarcerated hemorrhoid

目的探讨硫酸镁甘油湿敷在嵌顿痔水肿防治中的临床效果。方法将2004～2007年收治的72例嵌顿痔随机分为治疗组（手术前后采用44％硫酸镁甘油湿敷）和对照组（术前温水坐浴）。比较两组资料术前痔水肿的改变情况，术后72h肛缘水肿发生率及肛门疼痛的评分，住院时间及皮赘残留率。结果治疗组、对照组入院时痔水肿评分分别为（3．27±0．69）分，（3．20±0．68）分，差异无统计学意义（t=0．665，P〉0．05），术前半小时痔水肿评分分别为（2．22±0．75）分，（3．03±0、82）分，差异有统计学意义（t=-4．372，P〈0．01）；治疗组术后72h有8例（21．6％）发生肛缘水肿，而对照组有17例（48．6％）发生肛缘水肿，两组比较差异有统计学意义（X^2=5．763，P〈O．05）；治疗组和对照组术后72h肛门疼痛评分分别为（1．59±0．80）分，（2．17±0．82）分，两组比较差异有统计学意义（t=-3．019，P〈O．01）；治疗组和对照组住院时间分别为（10．38±1．73）d，（12．51±1．99）d，两组比较差异有统计学意义（t=-4．839，P〈O．01）；治疗组残留皮赘2例（5．4％），对照组残留皮赘9例（25．7％），差异有统计学意义（r=5．731，P〈o．05）。结论采用44％硫酸镁甘油湿敷水肿痔表面及术后伤口，对防治嵌顿痔手术前后水肿能起到较好的疗效。

Objective To explore the clinical effect of wet dressing with glycerine magnesium sulfate in preventing and treating oedema of incarcerated hemorrhoid. Methods A total of 72 cases of incarcerated hemorrhoid from 2004 to 2007 were randomly divided into two groups： therapy group was treated with 44% glycerine magnesium sulfate wet dressing perioperatively, while control group with warm sitz bath.Results Scores of oedema of incarcerated hemorrhoid in therapy group and control group at admission was 3.27±0.69 and 3.20±0.68, respectively （ P〉0.05）, while at 30 min before operation the oedema scores was 2.22±0.75 and 3.03±0.82, respectively （ P 〈0.01）. Eight cases（21.60% ） developed anus edge oedema in therapy group, while 17 cases（48.6% ）in control group, and there was significant difference（ P〈0.01）. Scores of anus pain at postoperative 72 h in therapy group and control group was 1.59±0.80 and 2.17±0.82, respectively and there was significant difference（ P 〈0. 01 ）. There was significant difference between therapy group and control group in length of hospital stay, which was 10.38±1.73 days and 12. 51±1.99 days, respectively. Two cases （5.4%）developed skin tag residual in therapy group and 9 cases （25.7% ）in control group and there was significant difference between the two groups（ P 〈0.01）. Conclusion It was useful to prevent and treat oedema of incarcerated hemorrhoid using 44% glycerine magnesium sulfate wet dressing at perioperative period.
