肝癌经导管动脉化疗栓塞术的CT灌注成像评价

The Evaluation of HCC Blood Supply after TACE Using CT Perfusion Imaging

目的:探讨CT灌注成像对肝细胞癌(HCC)经导管动脉化疗栓塞术(TACE)的临床意义。方法:对30例HCC患者于TACE前1d及术后7～10d行CT灌注扫描,测量肿瘤组织及正常肝组织灌注参数。结果:肿瘤组织TACE术前、术后HBF、HBV、HAP存在显著差异(P<0.05),MTT、PS、HAF、PVP无显著差异(P>0.05)。正常组织TACE术前、术后各灌注参数无显著差异(P>0.05)。结论:CT灌注成像可客观评价TACE前后血供变化,对指导TACE治疗有着重要价值。

Objiective To investigate the blood supply changes of hepatocellular carcinoma（HCC） before and after transcatheter arterial chemoembolization （TACE） using computed tomography perfusion imaging （CTPI）. Methods CT perfusion scans were performed 1 d before treatment and 7 - 10 d after TACE for 30 patients with HCC. The perfusion images and parameters of HCC lesions and normal tissues were measured, including the values of blood flow（BF）, blood volume （BV）, mean transit time （ MTT）, permeability （ PS ）, hepatic arterial fraction （ HAF）, hepatic arterial perfusion （ HAP）, portal vein perfusion（PVP） . Results The differences of HBF, HBV, HAP in HCC lesions were statistically significant, MTT, PS, HAF, PVP were not significant. There was not significant among the parameters in normal tissue before and after therapy. Conclusion CTPI may be a safe and effective methods to evaluate the therapeutic effect of TACE.
