摘要
目的探讨H.pylori相关性十二指肠溃疡与胃癌病例中NF-κB、Bcl-2、VEGF、IL-1β表达的差异及相互关系,进一步了解H.pylori致胃癌与十二指肠溃疡的发病机制。方法选取胃镜检查的胃黏膜活检标本,十二指肠溃疡患者33例,胃癌患者63例,通过免疫组化SP法测定NF-κB、Bcl-2、VEGF、IL-1β在H.pylori感染与非感染的胃癌及十二指肠溃疡患者胃黏膜中的表达情况。结果十二指肠溃疡组和胃癌组中,H.pylori感染者NF-κB、VEGF与IL-1β的表达水平高于H.pylori阴性者(P<0.01或0.05),Bcl-2的表达无显著差异(P>0.05);H.pylori感染病例中,NF-κB、Bcl-2、VEGF在胃癌组的表达水平高于十二指肠溃疡组(P<0.01或0.05),IL-1β的表达无显著差异(P>0.05);胃癌组与球溃疡组中,NF-κB的表达水平与VEGF均呈正相关(P<0.05);胃癌组中,NF-κB和VEGF的表达水平与Bcl-2呈正相关(P<0.05),与IL-1β表达水平无明显相关性(P>0.05);球溃疡组中,NF-κB和VEGF的表达水平与IL-1β表达水平呈正相关(P<0.05),与Bcl-2表达水平无明显相关性(P>0.05)。结论幽门螺杆菌感染可上调宿主胃黏膜上皮NF-κB、VEGF及IL-1β的表达,三者表达水平具有协同性,其中NF-κB、VEGF表达的差异与十二指肠溃疡与胃癌两种不同的疾病转归相关。
Objective To examine the expressions of NF-KB, VEGF, Bcl-2, IL-1β in H. pylori positive and nega- tive gastric mucosa of duodenal ulcer patients and gastric cancer patients, and to approach the influence of H. pylori in- fection on their expression in gastric mucosa, and the diversity between duodenal ulcer group and gastric cancer group. Methods Ninety-six biopsy specimens were collected, including 33 duodenal ulcer patients and 63 gastric cancer pa- tients. Expressions of NF-KB, VEGF, Bcl-2, IL-1β in H. pylori positive and negative gastric mucosa of duodenal ulcer patients and gastric cancer patients were detected by immunohistochemistry SP-method. Results In both duodenal ulcer and gastric cancer groups, expressions of NF-KB, VEGF, IL-1β were higher in H. pylori positive patients than that in H. pylori negative patients ; there was no significant difference between the expressions of Bcl-2 in H. pylori positive patients and in H. pylori negative patients. Expressions of NF-KB, VEGF, Bcl-2 were higher in gastric cancer group than that in duodenal ulcer group in H. pylori positive patients. In both gastric cancer group and duodenal ulcer group, there was a significant positive correlation between the expressions of NF-KB and VEGF (P 〈 O. 05). In gastric cancer group, ex- pressions of NF-KB and VEGF were significantly correlated with the expression of Bcl-2 (P 〈 O. 05) , but not correlated with the expression of IL-1β (P 〉 O. 05 ). In duodenal ulcer group, expressions of NF-KB and VEGF were significantly correlated with the expression of IL-1β (P 〈0.05) , but not correlated with the expression of Bcl-2 (P 〉0.05 ). Conclusion H. pylori can increase expressions of NF-KB, VEGF and IL-1β in host gastric mucosa. Their expressions are correlated with each other. In the pathogenic process of different gastro-duodenal diseases, there is a remarkable difference of the expressions of NF-KB, VEGF activated by H. pylori infection.
