肺癌患者外周血TA活性及CEA mRNA、CK19 mRNA的转录情况及其意义

Expression of CEA mRNA,CK19 mRNA and Telomerase Activity in Peripheral Blood of Patients with Lung Cancer and Its Clinical Significance

摘要 为了了解肺癌患者外周血细胞TA及CEA mRNA和CK19 mRNA的转录情况及其检测的临床意义,选取肺癌患者103例、肺良性病变患者(疾病对照组)67例和健康人(健康对照组)37名,采集外周血细胞,应用逆转录-套式-聚合酶链反应法(RT-NP-PCR)检测CEA mRNA和CK19 mRNA,端粒末端重复序列扩增-杂交-酶联免疫法检测TA。结果发现,肺癌组三种标志物阳性率均显著高于两个对照组(P<0.01),且CEA mRNA和TA的阳性率均显著高于CK19 mRNA(P<0.01)。小细胞肺癌CEA mRNA的阳性率显著低于其他类型肺癌(P<0.05),且显著低于TA(P<0.05)。结果表明检测血液CEA mRNA、CK19 mRNA转录和TA有助于发现肺癌细胞血液转移,其中CEA mRNA和TA的诊断价值高于CK19 mRNA;联合检测CEA mRNA和TA有助于鉴别小细胞肺癌。 To explore the clinical significance on the detection of Carcinoembryonic antigen mRNA(CEA mRNA), Cytokeratin 19 mRNA(CK19 mRNA) and Telomerase activity (TA) in peripheral blood of patients with lung cancer, blood samples were taken from 103 cases of lung cancer, 67 cases of benign pulmonary disease(disease control group)and 37 cases of healthy persons (healthy control group). The expression of CEA mRNA and CK19 mRNA in blood were detected by reverse transcriptase-nested primers-polymerase chain reaction. The TA in blood was detected by telomeroc repeat amplification protocol-hybridism-enzyme-linked immunosorbent assay. The results showed that the positive rates of CEA mRNA, CK19 mRNA and TA in the lung cancer group were significantly higher than that of the other two groups (P〈0.01), and the positive rates of CEA mRNA and TA were higher than that of CK19 mRNA(P〈0.01). The positive rate of CEA mRNA in small cell lung cancer was lower than that of the other lung cancer(P〈0. 05) and TA(P〈0. 05). The detection of CEA mRNA, CK19 mRNA and TA are more sensitive than CK19 mRNA. Combined detection of CEA mRNA and TA may be helpful to distinguish small cell lung cancer.
