踝臂指数、脉搏波传导速度对冠心病的预测价值 被引量:10

Value of ABI and BaPWV for predicting coronary artery diseases

摘要 目的评价踝臂指数(ABI)及肱-踝脉搏波传导速度(BaPWV)在冠心病高危人群对发生冠心病的预测价值。方法对114例冠状动脉造影(CAG)患者行ABI及BaPWV测量、病史采集及常规化验检查,评价ABI及BaPWV对冠心病高危人群发生冠心病的预测价值。结果冠脉病变组与非病变组之间ABI、BaPWV有明显差异(P<0.05);114例患者分别依据ABI及BaPWV四分位数分为4组,ABI最低组发生冠心病的危险性显著升高,而BaPWV最低组则发生冠心病的可能性显著减小。结论ABI及BaPWV均可反映冠脉病变的程度;ABI降低是冠心病独立的危险因子,而BaPWV低则是非冠心病有意义的独立预测因子。 Objective To compare the applicability of ankle-arm index(ABI) and brashial-ankle pulse wave velocity(BaPWV) as the markers for predicting the prevalence of coronary atherosclerotic heart disease (CHD) in Chinese subjects at high risk of CHD. Methods We studied 114 consecutive patients who had taken coronary angiography or percutaneous coronary intervention. All patients underwent ABI, BaPWV and serum factors evaluation in addition to history collection. The applicability of ABI and BaPWV as the markers for predicting the prevalence of CHD in subjects at high risk of atherosclerotic cardiovascular disease was compared, Results ABI was significantly lower in subjects with CHD than that in those without CHD (P〈0. 01). The BaPWV was significantly higher in subjects with CHD than that in those without CHD (P〈0. 01), The lowest ABI quartile was found to be a significant independent marker of the presence of CHD, whereas the lowest BaPWV quartile was found to be a significant independent marker of the absence of CHD. Conclusion BaPWV and ABI can reflect the extent of coronary atherosclerosis lesions, A low ABI is an independent marker of the presence of CHD in subjects at high risk of CHD, whereas a low brachial-ankle PWV may also be used as an independent marker of the absence of CHD in these patients.
