彩色多普勒超声诊断剖宫产子宫瘢痕部位妊娠的价值

Diagnostic value of color Doppler ultrasound for pregnancy in a previous caesarean section scar

目的探讨彩色多普勒超声诊断剖宫产子宫瘢痕部位妊娠的临床价值。方法对中国福利会国际和平妇幼保健院2005年5月—2007年4月收治的21例剖宫产子宫瘢痕部位妊娠的二维及彩色多普勒超声表现进行回顾性分析。结果21例患者中,14例为二维声像图示胎囊型,7例为混合包块型。20例经阴道彩色多普勒超声显示血流丰富,1例血流不明显。20例超声明确诊断为剖宫产子宫瘢痕妊娠,1例误诊,超声诊断符合率为95%。结论超声检查尤其是经阴道彩色多普勒超声是剖宫产子宫瘢痕妊娠的最佳检查方法之一。

Objective To evaluate value of color Doppler in the diagnosis of the pregnancy in the scar of previous caesarean section. Methods Two diemensional （2D） and color Doppler flow imaging （CDFI） ultrasonograms of the 21 patients with pregnancy in previous caesarean section scars （May 2005 to April 2007） were retrospectively analyzed. Results All cases were verified by clinical and pathologic results, including 14 with gastrula type and 7 with commix mass type. Rich blood supply was observed in 20 cases by transvaginal color Doppler ultrasound, and the blood ,signals were weak in 1 case. Twenty cases were correctly diagnosed by ultrasound and there was 1 misdiagnosis, with the correct diagnosis rate of 95%. Conelusion Ultrasonography, especially transvaginal color Doppler ultrasound, is one of the best methods for the diagnosis of the pregnancy in the uterine sear of previous caesarean section.
