部队无偿献血者RhD阴性血液冷冻血库的建立与临床应用

Establishment and Clinical Application of Rhesus Negative Frozen Blood Bank from the Military Volunteer Donors in Beijing Area

目的探索驻京部队无偿献血者Rh血型分布规律,建立无偿献血者RhD阴性血液冷冻血库并应用于临床。方法采血后留取标本,对献血者进行RhD血型鉴定,对初筛RhD阴性标本进一步用抗-C、抗-c、抗-E、抗-e进行Rh血型分型,建立RhD阴性献血者资料库并定期进行更新维护。对检出的RhD阴性血液分别制备成悬浮红细胞、冰冻红细胞、新鲜冰冻血浆和新鲜冰冻血小板,应用于RhD阴性患者。结果2000～2006年共筛查献血者86255例,检出RhD阴性献血者283例(0.3281%),其中O型102例,A型79例,B型81例,AB型21例。在RhD阴性献血者中ccdee和Ccdee两种表现型约占85%。7年来共向临床患者供应RhD阴性血液269U。结论通过在驻京部队官兵中筛查RhD阴性无偿献血者并建立RhD阴性血液冷冻血库,不仅可有效保证临床患者对RhD阴性血液的需求,且在应对突发事件及战时血液卫勤保障方面具有重要意义。

Objective To explore the distribution of rhesus group in military volunteer donors in Beijing area,and to establish rhesus group negative frozen blood bank and supply the negative blood to recipients. Methods The RhD typing were performed after blood collection. The negative samples were typed with anti-C,anti-c,anti-E and anti-e reagents. We contacted with the RhD negative donors to get their personal data such as： address, photo, the place where they joined in the army and phone number. All the data was put into computer to establish a negative donor bank,which was renewed periodically. The blood was separated into such components as： red blood cell suspension, cryopreserved erythrocytes, fresh frozen plasma and fresh frozen platelet supplied to clinical RhD negative recipients. Results From the year 2000 to 2006, altogether 86 255 donors were screened. As a result, 283 RhD negative donors were confirmed （0. 328 1%）, of whom the ABO distribution were 102 O,79 A,81 B and 21 AB. Among the negative donors, the types of ccdee and Ccdee had a proportion of 85%. Altogether 269 units of negative blood were supplied to recipients during 7 years. Conclusion The screening of RhD in military volunteer donors in Beijing area and establishment of rhesus negative frozen blood bank could not only meet the clinical transfusion needs but also be of great importance to meeting emergencies and the rare blood logistics guarantee in case of the war.
