腹腔镜胃癌根治术的临床应用

The application of laparoscopic radical gastrectomy

目的:探讨腹腔镜胃癌根治术的可行性和效果。方法:为20例胃癌患者行腹腔镜胃癌根治术,其中根治性远侧胃次全切除术18例;根治性全胃切除术2例。结果:1例中转开腹,1例改行经胸食管空肠吻合,余者行腹腔镜胃癌根治术均获成功。平均手术时间270min(210～380min),平均出血量185ml(80～350ml),平均清扫淋巴结21.5枚(13～29枚),平均肛门排气时间2.5d(1～4d)。结论:腹腔镜胃癌根治术是安全可行的,能获得与开腹手术相当的根治效果。

Objective：To explore the feasibility, and effect of laparoscopic radical gastrectomy. Methods：Among the 20 cases with gastric cancer, 18 cases received radical distal gastrectomy,2 cases received total gastrectomy. Results：Eighteen cases underwent laparoscopic radical gastrectomy successfully, but one case conversion to open procedure, 1 case converted to open chest for esophagojunostomy. The mean operation stay was 270min（210-380min） ,the mean volume of blood loss was 185ml（80-350ml） ,the mean number of harvested lymph nodes were 21.5 （ 13-29）, the mean time was 2.5 days （ 1 4days）for gastrointestinal function recovery. Conclusions： Laparoscopic radical gastrectomy is a feasible, safe and minimally invasive treatment and can achieve the same outcome as the open operation.
