E选择素及其基因多态性与川崎病 被引量:2

The relationship between E-selectin gene polymorphism and Kawasaki disease

摘要 川崎病是全身性非特异性血管炎的发热性疾病,多侵犯冠状动脉,目前已取代风湿热成为儿童后天性心脏病最常见的原因。非特异性血管炎由内皮细胞活化介导,活化的内皮细胞表达E选择素,导致白细胞边集并穿过内皮细胞,同时被激活的白细胞产生大量的生物活性产物,进一步激活E选择素。川崎病患儿急性期、亚急性期和恢复期体内血浆E选择素水平明显增高,免疫效应细胞向血管局部浸润,导致冠状动脉损害。采用E选择素表达抑制剂或功能阻断剂(如丙种球蛋白)可明显减少白细胞在微静脉壁的黏着数,降低冠状动脉的损害。中国汉族人群中携带E选择素SR型基因患者E选择素血清水平显著高于Ss基因型患者,通过影响E选择素在内皮细胞上的表达,进而影响川崎病的发生和发展。
