纤维乳管镜在乳管炎及乳管扩张症中的诊断及治疗价值

Clinical Value of Fiber Ductoscopy in Diagnosis and Treatment of Galactophoritis or Mammary Duct Eectasia

目的评价纤维乳管镜(FDS)在乳管炎及乳管扩张症中的诊断及治疗价值。方法2005年11月—2008年1月,对98例乳头溢液患者行FDS检查,并对其中55例非占位性病变患者行术中乳管冲洗、庆大霉素及地塞米松灌注治疗。结果55例非占位性病变患者中,单侧41例、双侧14例,溢液为血性者11例,乳白色者7例,淡黄色者37例。FDS检查拟诊为乳管扩张症7、乳管炎33例、乳管炎合并乳管扩张症15例。55例患者经乳管冲洗、庆大霉素及地塞米松灌注治疗后47例溢液消失,有效率为85.5%。结论FDS检查应用于乳管炎及乳管扩张症的诊断及治疗方便,安全,准确。

Objective To evaluate the clinical value of fiber ductoscopy （FDS） in diagnosis and treatment of patients with galactophoritis and mammary duct ectasia （MDE）. Methods From November 2005 to January 2008, 98 patients with nipple discharge were submitted to FDS examination. Fifty - five of them with non - occupying lesions were given treatment of duct cleaning, and gentamicin and dexamethasone perfusion during operation. Results Of the 55 patients with non - occupying lesions, 41 presented with unilateral lesions, 14 with bilateral, 11 with bloody nipple discharge, 7 with ivory milk and 37 with light yellow milk. Seven were diagnosed as having mammary duct ectasia, 31 as galactophoritis, and 15 as mammary duct ectasia with galactophoritis. Discharge disappeared in 47 after treatment of duct cleaning, and gentamicin and dexamethasone perfusion, with an effective rate of 85.5%. Conclusion FDS is a convenient, safe and accurate methed in diagnosis and treatment of galactophoritis and MDE.
