前后路联合术配合多功能引流管治疗多节段下腰椎结核

Anterior and posterior combined operation accompanied UTL for treatment of multiple segment tuberculosis of lower lumbar spine

目的探讨一期前后路联合手术治疗下腰椎结核的临床疗效及适应证。方法对41例下腰椎结核患者采用一期前路病灶清除、椎体间植骨加后路椎弓根钉内固定术配合多功能引流管（UTL）引流渗血、局部注药治疗,术后随访6月-2年。结果本组41例植骨块均骨性融合,神经功能也有不同程度恢复,无局部复发病例。结论一期前路病灶清除、椎体间植骨加后路椎弓根钉内固定术,术后配合UTL治疗下腰椎结核,能彻底清除结核病灶,保证脊柱的稳定性,促进植骨块的骨性融合及病灶愈合。

Objective To evaluate the clinical effect and indications of anterior focus elimination, interbody fusion procedure and posterior transpedicular screw system internal fixation for treatment of lower lumbar tuberculosis. Methods 41 lower lumbar tuberculosis patients treated with anterior focus elimination, interbody fusion procedure and posterior transpedicular screw system internal fixation were followed up for 6-24 months. Results All the patients showed successful interbody fusion,neural function recovery with different degrees, and no recrudescence. Conclusion Anterior focus elimination,interbody fusion procedure and posterior transpedicular screw system internal fixation show valuable in strengthening the stability of the spine, providing successful internal fusion and healing of the tuberculosis focus.
