摘要 目的:初步研究前列腺增生患者逼尿肌肾上腺素能α1a/α1d受体亚型mRNA的表达及其作用。方法:选择前列腺增生膀胱出口无梗阻患者(对照组)9例和膀胱出口梗阻患者(实验组)21例,根据尿动力学将实验组分为逼尿肌稳定组和不稳定组,采用半定量RT-PCR方法检测三组逼尿肌α1a/α1d受体亚型mRNA的表达,采用离体逼尿肌拉力实验检测α1受体激动剂对三组逼尿肌收缩力的影响。结果:前列腺增生患者逼尿肌肾上腺素能受体α1d亚型约占α1受体2/3;对照组、逼尿肌稳定组和不稳定组α1d受体mRNA相对含量为7.53±1.08、7.77±1.50和11.64±3.80,不稳定组α1d受体明显增加(P<0.05);α1受体激动剂诱导逼尿肌收缩力呈浓度依赖性,对不稳定组作用大于稳定组和对照组(P<0.05),对照组与稳定组没有差别。结论:α1受体激动剂能够显著增加膀胱出口梗阻患者逼尿肌收缩,逼尿肌肾上腺素能受体α1d亚型可能参与逼尿肌收缩力的变化。 Objective:To investigate the mRNA expression of bladder α1a/α1d adrenoceptor subtypes and function in patients with benign prostatic hyperplasia (BPH). Methods: 9 patients witl^out bladder outlet obstruction (Control) and 21 patients with bladder outlet obstrction (BOO) deprived from BPH were involved. According to the results of urodynamic test, patients with BOO were divided into detrusor stability group (DS) and detrusor in- stability group (DI). A half-quantitative reverse transcriptase-polymerase chain reaction based method was used to quantify α1a/α1d subtype mRNA expression. The contractile response to α1-AR agonists was examined using an in vitro detrusor srip contraction technique. Results:ala-AR was major subtype of α1-AR. Among three groups. The α1d-AR expression level was 7.53±1.08、7.77±1.50 and 11.64±3.80 respectively. The expression level of α1d- AR in DI was increased(P〈0.05). The α1-AR agonists induced detrusor strip contraction was in dose dependant manner. The maximum contraction in DI was significantly higher than in Control and in DS, the maximum contraction was the same between in control and in DS. Conclusions:The α1-AR agonists can induce detrusor contraction of BOO associated with BPH, bladder ald adrenoceptor subtypes may play a role in detrusor contraction response to α1-AR agonists.
