摘要
目的探讨肝癌氩氦刀冷冻治疗对丙泊酚全凭静脉麻醉(TIVA)深度的影响。方法100例ASAⅠ~Ⅱ级肝癌患者择期在丙?白酚全凭静脉麻醉下行氩氦刀冷冻治疗术,随机分为A、B两组各50例。A组维持脑电双频指数(BIS)在同一水平,B组丙泊酚靶控浓度保持恒定,记录各时间段血压(BP)、心率(HR)、血饱和度(SPO2)、体温(T)、BIS值、丙泊酚用量及麻醉苏醒时间。结果A组丙泊酚用量减少、苏醒快(P〈0.05);B组冷冻期体温下降、BIS值显著降低、血压下降、心率减慢、苏醒时间延长(P〈0.01)。结论肝癌氩氦刀冷冻治疗术中体温降低可引起麻醉深度改变及丙泊酚用量减少。
Objective To investigate the effects caused by cryosurgery for liver cancer on depth of anesthesia with propofol TIVA. Methods Forty ASA Ⅰ - Ⅱ patients scheduled for cryosurgery of liver cancer were randomly divided into two groups : A ( n = 50) and B (n =50). BIS levels in the group A were maintained at the same level, the target concentration of propofol in the group B were kept constant. BP, HR, SPO2, T and BIS were recorded at deferent time. The dosage of propofol and awaking time were recorded. Results The dosage of propofol in group A was lower than that in group B, and the awaking time in group A was significantly shortened ( P 〈 0. 01 ) ; The T, BIS level, BP, HR in the group B decreased significantly ( P 〈 0.05 ), and the awaking time was prolonged ( P 〈 0. 01). Conclusion The hypothermia caused by cryosurgery for liver cancer could strengthen the depth of anesthesia and decrease the dosage of propofol.
