直肠癌远侧端浸润的PET/CT显像与分子病理学研究 被引量:1

摘要 目的:探讨直肠癌远侧端浸润PET/CT成像及其分子病理学改变,为直肠癌保肛手术提供评估价值.方法:对45例中低位直肠腺癌下缘进行标记;PET/CT检查并结合免疫组织化学方法对直肠癌的远侧端黏膜p53及CD34表达进行了研究.结果:直肠癌远侧端PET阳性段长度为4~12mm,平均为7.0mm;PET阳性段与阴性段p53与微血管密度有显著性的差异,PET阳性段p53与CD34均为高表达;直肠远侧端PET阳性段长度与肿瘤的大小、病理类型以及肿瘤分期无关.结论:肿瘤的PET/CT显像不但与肿瘤组织有关,而且还与肿瘤的癌旁组织有关,直肠癌远侧端PET/CT显像将为直肠癌的临床手术提供依据并对评估保肛手术具有一定的指导意义. AIM: To study PET/CT imaging and molecular phatological changes of distal tissue to rectal cancer and to explore its value in sphincter reserving operation of rectal cancer. METHODS: The 45 cases of the rectal cancer were marked for the cancer anal margin before operation, and then the cases were detected by PET/CT imaging; p53 and CD34 expression in rectal distal tissue portion were detected by using immunohistochemical techniques. RESULTS: The distance from cancer anal margin to the distal positive portion was 4-12 ram, averagely 7.0 mm. Expression of p53 and CD34 in PET/CT positive portion was significantly higher than that in PET/CT negative portion. The length of distal PET positive portion was not associated with size, kinds and stages of rectal cancer. CONCLUSION: The distal tissue to cancer margin could be revealed by PET/CT imaging, which might be of significance in sphincter reserving operation of rectal cancer.
