鼻内镜下微填塞法治疗老年性鼻出血

Endoscopic Microinvasive Nasal Packing for Elder Epistaxis

目的探讨鼻内镜下微填塞法在老年性鼻出血的临床应用。方法对我院2002年4月-2006年4月间收治的43例老年性鼻出血病例的临床资料进行回顾性分析。结果43例病例中,仅3例患者无基础性疾病。其中,40例伴高血压,10例同时伴有糖尿病。43例患者中有4例伴有鼻中隔偏曲。上述病例中,出血部位以嗅裂、下鼻道后端、中鼻道以及筛顶为主。伴有鼻中隔偏曲患者因内镜无法探查鼻腔,先行局麻下鼻中隔矫正术。所有病例均在鼻内镜下行明胶海绵微填塞。结论鼻内镜下微填塞法治疗老年性鼻出血可准确、迅速查找出血点,根据部部位进行微填塞,减轻患者痛苦,避免盲目填塞造成止血失败或新的出血。

Objective： To explore the clinic application of endoscopic microinvasive nasal packing for elder epistaxis. Method： Analysis retrospectively of the clinic data for the 43 elder epistaxis cases in our hospital from May,2002 to May,2006. Result： in these 43 cases, 3 cases have no other desease,40 cases compared with hypertension and in these 40 cases there are 10 cases compared with diabetes. In these 43 cases,4 cases compand with deviation of nasal septum. The common areas in case of these elder epistaxis is olfactory cleft, the posterior part of the inferior meatus, middle meatus and the crown of ethmoid bone. For these deviation of nasal septum cases, we did correction of nasal septal in local anesthesia because we can＇t check them in endocopy. All of these cases were microinvasive packinged with gelfoam. Conclusion： We can searched the bleeding points precisely and rapidly with endoscopic microinvasive nasal packing for elder epistaxis. This endoscopic microinvasive nasal packing according to the bleeding points can relieve pain of painters and can avoid blindly packing create defeat hemostasis or the new hemorrhage.
