咽旁间隙肿瘤手术方法和径路的选择

Surgical management and selection of approaches for parapharyngeal space neoplasms

目的探讨咽旁间隙肿瘤手术方法和入路的选择。方法对6例原发于咽旁间隙较大的良性或低度恶性肿瘤的手术治疗和效果进行回顾分析。结果分别采取颈侧入路、腮腺后入路、口咽软腭部入路及经颈颌入路的方法,完整切除了咽旁间隙肿瘤。结论根据肿瘤的起源、大小、部位和性质选择咽旁间隙肿瘤的手术入路。对于比较大的良性实体或低度恶性肿瘤以及恶性肿瘤,以颈外入路和颈颌入路为较好的选择。

Objective To discuss the methods and surgical approaches for neoplasms in the parapharyngeal space. Methods Clinical data of 6 cases with huge primary neoplasm in the parapharyngeal space was analyzed retrospectively. Results According to the size and location of the parapharyngeal spacetumors, the neoplasms were fully resected via lateral cervical approach, posterior parotid gland approach, oral and soft palate approach, or cervical approach with mandibular swing respectively. Conclusion The approach to the primary neoplasm in parapharyngeal space should be chosen according to its origin, size, position, and characteristics. It is considered that the lateral cervical approach or the cervical approach with mandibular swing can provide a safe and efficient removal of the primary parapharyngeal space neoplasm.
