摘要
目的:探讨大动脉弹性与冠心病严重程度的相关性。方法:武警总医院心血管内科病房因拟诊为冠心病者123例,按照冠状动脉造影结果分为冠脉正常组、1支病变组,2支病变组,多支病变组;造影前测定颈-桡脉搏波速度(CR-PW V)、颈-股脉搏波速度(CF-PW V),作为评估大动脉弹性的指标。结果:(1)冠脉造影阳性者的CR-PW V、CF-PW V较冠脉造影正常者显著增高(P<0.05)。CF-PW V在1支病变组[(11.56±1.99)m/s],2支病变组[(12.49±1.21)m/s],多支病变组[(14.14±3.18)m/s]组间相比均差异有显著性(P<0.05),CR-PW V随病变加重仅有较弱递增趋势(P>0.05)。(2)预测冠心病的最佳指标是CF-PW V、年龄、性别、高脂血症、糖尿病。CR-PW V最终未进入回归模型。结论:(1)CF-PW V是冠心病的独立相关因素;(2)CF-PW V与冠状动脉病变严重程度呈正相关。
Objective:To evaluate whether there was any correlation with coronary artery disease severity. Methods : 123 consecutive patients who were diagnosed coronary heart disease (CHD) at the Cardiology Department of Gener al Hospital of Armed Police Forces. underwent a coronary angiography. According to the results,they were classified into normal group and 1-, 2-,or Multiple vessel disease group. CR-PWV, CF-PWV were measured as the indexes of large artery distensibility on the day before coronary angiography. Results: (1)Levels of CR PWV and CF PWV were signifi cantly higher m coronary heart disease group than those in normal group (P〈0. 05). CF-PWV increased significantly(P〈0.05) as the number of stenotic vessels increased :1-vessel(11.56±1. 99 m/s,2-vessel(12.49±1.21m /s or multiple vessel(14. 14±3. 18 m/s. There was a weak incresing tendency of CR PWV (P〉0.05) (2) CF PWV,age,sex,hyperlipemia.diabetes were the main determinants for coronary artery disease. While CR-PWV was excluded from the regression model. Conclusions:(1 )CF-PWV is an independent correlation factor of coronary artery disease. (2)CF-PWV was positively correlated with coronary artery disease severity.
