综合性疗法治疗慢性精神分裂症的疗效

The therapeutic effect of combined therapy treating chronic schizophrenia

目的探讨综合性疗法辅助药物治疗慢性精神分裂症的疗效及其影响因素。方法将106例诊断均符合中国精神疾病分类方案与诊断标准CCMD-3的慢性精神分裂症患者随机分为观察组和对照组各53例,两组维持药物治疗,观察组行综合疗法,对照组行常规工娱疗法。治疗前及治疗后3个月应用简明精神病评定量表(BPRS)、护士用住院患者观察量表(NOSIE)、住院精神病患者社会功能评定量表(SSPI)、自知力与治疗态度问卷(ITAQ)、自知力评定量表(SAUMD)综合评价患者的疗效情况。结果两组治疗后BPRS、NOSIE、SSPI、ITAQ、SAUMD评分改变,与治疗前比较,差异有统计学意义(P<0.01),减分率差异也有统计学意义。多元逐步回归分析,治疗后得分与治疗次数呈负相关,与活动能力、配合治疗呈正相关。结论综合性疗法有较好的效果,更能改善患者症状,提高其生活自理能力和社会适应能力以及促进自知力恢复。

Objective To explore the therapeutic effect of combined therapy treating chronic schizophrenia and its influential factors. Methods 106 patients with chronic schizophrenia who met the criteria of Chinese Classification and Diagnostic Criteria of Mental Disorders 3 （CCMD-3） were randomly divided into study group （n= 53,on the basis of assisted drug treatment,combined therapy given） and control group （n= 53 ,on the basis of assisted drug treatment,occupation recreational therapy given）. At prior treatment and post-treatment 3 month,Brief Psychiatric Rating Scale （BPRS） ,Nurses Observation Scale for Inpatient Evaluation （NOSIE） ,Scale of Social Function in Psychosis Inpatients （SSPI）, Insight and Treatment Attitudes Questionnaires （ITAQ） and Scale to Assess Unawareness of Mental Disorder （SAUMD） were applied to comprehensively evaluating the therapeutic effect. Results There were significant differences in BPRS,NORS,SSPI,ITAQ and SAUMD grading score as well as score subtraction rates between before and after treatment. Multielement stepwise regression analysis showed that the score after treatment was negatively correltive to treatment frequency,while positively correlative to locomotor activity and treatment compliance. Conclusion Combined therapy is effective to amelioration of patients＇ symptoms,improvement of self-care ability and social adaptability,as well as insight recovery.
