脐带先露及脐带脱垂65例临床分析

Clinical analysis of 65 presentation or prolapse of umbilical cord cases.

目的探讨脐带先露及脐带脱垂与围生儿结局的关系。方法回顾性分析65例脐带先露及脐带脱垂的临床资料。结果脐带先露组剖宫产占80.49%(33/41),显著高于脐带脱垂组的54.17%(13/24)(P<0.05)。新生儿1minApgar评分脐带先露组与脐带脱垂组存在极显著差异(P<0.001),新生儿5min Apgar评分两组亦存在显著差异(P<0.05)。脐带先露组围生儿死亡率2.44%(1/41),明显低于脐带脱垂组20.83%(5/24)(P<0.05)。结论围生儿结局脐带先露组明显优于脐带脱垂组,早期诊断和干预脐带先露,防止脐带脱垂的发生,对改善围生儿结局有重要的作用。

Objective： To evaluate perinatal outcomes of pregnancies complicated by presentation or prolapse of umbilical cord. Methods ： 65 cases with presentation or prolape of umbilical cord weFe reviewed retrospectively. Results ： The rate of cesarean section in the cord presentation group 80.49% （33/41）, was significantly higher than the rate in the cord prolapse group 54. 17% （13/24） （ P 〈 0. 05）. Between the two groups, newborns Apgar scores at 1 and 5 min had some significant differences （ P 〈 0. 001, P 〈 0.05）. The perinatal mortality rate of the cord presentation group 2.44% （ 1/41 ）, was significantly lower than of the cord prolapse group 20. 83% （5/24） （ P 〈 0. 05 ）. Conclusion： Perinatal outcomes of the cord presentation group were better. Early diagnosis and prompt intervention can prevent cotd presentation from prolapse to result in a satisfactory outcome.
