贲门胃底癌MSCT增强扫描及多平面重组的临床应用价值

Clinical Application of Multi-slice CT Enhancement and Multi-planar Reformation in the Diagnosis of Gastric Cardia Carcinoma

目的:探讨MSCT增强扫描及多平面重建对贲门胃底癌临床应用价值。方法:48例经胃镜诊断为贲门胃底癌并经手术证实的患者行MSCT平扫、增强双期扫描及多平面重建,分析其CT表现。结果:局限贲门区5例;贲门胃底区43例,其中侵犯下段食管12例,侵至胃体小弯侧14例,胃体及胃窦均受累6例。病灶厚度11～42 mm,中位厚度20 mm。病灶分层强化41例,单层强化7例。周围淋巴结转移26例,肝脏转移5例。浆膜侵犯9例。结论:MSCT增强扫描及多平面重建对贲门胃底癌诊断效果良好,具有重要的临床应用价值。

Objective：To investigate the clinical application of multi-slice CT （MSCT） enhancement and multi-planar reformation （MPR） in the diagnosis of gastric cardia carcinoma. Methods： 48 patients with gastric cardia carcinoma proved by gastroscopy and surgery underwent MSCT pre-contrast and biphasic （arterial phase and portal venous phase） contrast scanning as well as MPR, the CT appearances of gastric cardia carcinoma were analyzed. Results： Only 5 patients had tumor just involving gastric cardia. The remaining 43 patients had tumor involved gastric cardia and fundus,of them,additional in volvement of lower esophagus was assessed in 12 patients,involvement of lesser curvature of stomach in 14 patients and gastric body as well as gastric antrum invasion in 6 patients. The thickness of tumor invasion ranged from 11 -42mm with the median as 20mm. There were 41 cases showing multi-layer and 7 cases showing monolayer enhancement. 26 cases had adjacent lymphatic metastasis,5 cases had liver metastasis and 9 cases had gastric serosa invasion. Conclusion： MSCT contrast enhancement and MPR provided helpful information for the diagnosis of gastric cardia carcinoma,and showed important application value in the clinical practice.
