革兰氏阴性菌肺炎低IgM血症患者血浆内毒素和血清CRP浓度的测定

DETERMINATION OF PLASMA ENDOTOXIN AND SERUM CRP IN PATIENTS WITH GNB PNEUMONIA AND CONCOMITANT LOW SERUM IgM

测定２１例革兰氏阴性菌（ＧＮＢ）肺炎血清ＩｇＭ低下患者的血浆内毒素和血清ＣＲＰ浓度，并与２０例ＧＮＢ肺炎ＩｇＭ增高患者对照比较，结果显示：ＩｇＭ低下组的血浆内毒素和血清ＣＲＰ浓度均显著高于ＩｇＭ增高组（Ｐ均＜０．０１），血清ＣＲＰ浓度与血浆内毒素浓度呈正相关；血清ＣＲＰ和血浆内毒素浓度均与血清ＩｇＭ浓度呈负相关。提示血清ＩｇＭ可中和血浆内毒素，在ＧＮＢ肺炎时血清ＩｇＭ低下会加重内毒素对机体的损害。

The levels of plasma endotoxin and serum CRP in 21 gramnegative,bacterial(GNB) pneulmonia with concomitant low serum IgM patients,and 20 GNB pneumonia with conconcomitant high serum IgM patients were determinedIn the low serum IgM group the levels of plasma endotoxin and serum CRP were significantly higher than those of the high IgM group (Pall<001)There was a positive correlation between the concentrations of serum CRP and plasma endotoxin(r=0815,P<001),but a negative correlation between the concentrations of serum CRP and IgM(r=-0645,P<001),which was also frue between the concentrations of plasma endotoxin and IgM (r=-0425,P<001)The result shows that serum IgM can neutralize plasma endotoxinThus,in GNB pneumonia patients,if serum IgM is low,the histological lesions may be exacerbated by the unchallenged endotoxin
